2023 TUITION

1. Children ages 6 weeks to 12 years are accepted in the Holy Family Early Learning Center.

2. There will be no discrimination by race, color, sexual orientation, country of origin, or creed. Upon enrollment, it is recommended that parents complete the intake form to provide staff with a better understanding of languages spoken in the home as well as dietary restrictions for personal, religious beliefs or family preferences. By filling out the intake form we are better able to keep routines as consistent as possible for each child. Parents are asked to complete a Family Intake form to provide additional information about the child’s background.

3. Prior to a child’s admittance to the Center, the following information must be completed and returned:

• Child Care Enrollment form

• Intake form (for children under 2 years)

• Health History and Emergency Care Plan

• Immunization Record

• Child Health Report

• Arrival/Departure Contract

• Release of Pictures form

The Physical/Health form shall be completed by a physician dated within one year prior or 90 days after admission for preschool children and not more than 6 months’ prior for infants and toddlers.

1. Your child will be accepted into the program when all forms are on file.

2. A registration fee of $75 and a supply fee of $75 will be charged to your account and will need to be paid prior to your child’s start date.

3. A supply fee of $75 will be charged each year and will be added to your bill through ProCare at the beginning of each contract renewal.

4. This fee will also apply to summer/school-age children.
