LIABILITY FORM

Please review the 2023-24 Faith Formation Handbook and Digital Communications Policy documents located at the bottom of this page.
You are required to fill out and submit the liability form in order for your child(ren) to participate in the Faith Formation program. Please use the link above, or download the form located at the bottom of this page.
 

PAYMENT 

Make online payments for 2023-24 Faith Formation tuition and fees here.
You will be able to select from two options:
GIVE NOW - payment in full
PLEDGE TO GIVE - payment plan

We also accept payments in person or by mail. Checks should be made payable to Holy Family Parish. Please include "Faith Formation Registration" in the memo.

$125/child (3+ children: $375 total) 
For tuition paid in full on or before August 31, 2023.
$150/child (3+ children: $450 total)
For tuition paid in full after August 31, 2023.

Add additional $50 for each:

• First Reconciliation & Communion student (grade 2 or upon request)
• Confirmation student (grade 11)

We offer free tuition for one child if you serve as a Catechist/SGL.
Scrip credits also apply toward tuition.
We offer financial aid for those who qualify. Please find the attached Tuition Assistance Form at the bottom of this page.
